Class of 2027 Kickoff to Senior Year Bonfire Registration Form

The Hopkinton High School (“HHS”) Bonfire Night is open to all students in the Class of 2027. There is a lot of very IMPORTANT information in this registration form, along with a number of agreements that require electronic signature authorization. PLEASE read all of the information CAREFULLY!

REGISTRATION

  1. Tickets purchased are NON-REFUNDABLE and NON-TRANSFERABLE.

  2. Students cannot register themselves even if over 18. A parent/guardian must complete the registration form, agree to all conditions, and discuss the entire registration form and agreements with the student attending. Digital signatures of both students and parents/guardians will be required before ticket is purchased.

  3. For those families registering multiple students for the Bonfire Night, the parent/guardian will need to complete a separate online Registration form and process a separate payment for each student.

The following forms of payment are accepted: Visa, Mastercard, Discover, American Express, Debit Card.  For online payments, a small processing fee will be added to the ticket cost.  If you need to pay by check, please contact hopkintonclass2027@gmail.com.  We want all students to be able to participate. If you are experiencing financial hardship, please contact your student's guidance counselor. They will provide a code for registration to honor privacy.

Allergies/Dietary Issues


Please be advised that the food at Bonfire Night will be provided by Juliana's Catering, BoMain and Uhlman’s. We cannot guarantee the prepared food does not contain allergens or have come into contact with items containing allergens. There will be packaged snacks available for students with gluten, dairy, and nut allergies.


Rules & Expectations


Students must comply with Hopkinton High School policies and procedures, all student behavior and discipline guidelines set forth in the HHS Student Handbook 2025-2026, state and federal regulations and laws, and the specific rules set forth below (collectively “Rules & Expectations”):

  1. Dress Code:  The regular school day dress code applies to Bonfire Night.

  2. Tobacco, Alcohol, Drugs, Controlled Substances, Drug Paraphernalia:  The use, possession, consumption, sale, purchase and/or distribution of tobacco, alcohol, drugs, narcotics, drug paraphernalia, vaping products, medication for illicit purposes and other illegal substances is prohibited at the Bonfire Night. Students deemed to be in possession or under the influence of drugs or alcohol will be denied entrance or removed immediately and face disciplinary consequences.  In the event a student is suspected of being under the influence of any of these substances, parents/guardians will be notified and responsible for immediate transportation of their student from Bonfire Night.
     
  3. Attendance:  In order to attend Bonfire Night, students must follow school attendance policies for events.  A student may be denied admittance if they are not in good standing at school at the time of Bonfire Night (e.g., suspended, expelled, unexcused absences). 

  4. Bags: No bags or backpacks will be permitted at the event.
  5. Early Departure:  Students that enter Bonfire Night are allowed to leave before the end of the event; however, once a student exits the event, re-entry will be denied.
     

  6. Discipline
    • Parents/guardians will be notified if any rules are violated. In the event that a student violates a rule set forth herein or in the student handbook, the student may be denied admittance or asked to leave the event. In addition, the student will be subject to any applicable consequences outlined in the HHS Student Handbook 2025- 2026, including without limitation, suspension, loss of privileges, expulsion, and loss of privilege to join in school events (such as senior week events and graduation).
    • Law enforcement may be notified of any alleged criminal violations.
    • The onsite chaperones will, in their sole and exclusive discretion, assess the appropriateness of student behavior.

Agreements


ACKNOWLEDGMENT & ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT

In consideration of the participation in the Bonfire Night of the student (“My student”), My student and I acknowledge and willingly agree that: 

  1. My student agrees to comply with all Rules & Expectations set forth above and understands that failure to do so will result in discipline, as described above.

  2. My student and I acknowledge and understand that the Bonfire Night involves risk of injury, illness, loss, and/or damage, including but not limited to personal injury (death and/or disability); bodily injury; exposure to communicable diseases, viruses, bacteria or illnesses; property damage and/or costs due to my student’s own actions, inactions or negligence, or the actions, inactions or negligence of others, and my student and I knowingly and freely assume all risks, both known and unknown, even those arising from the negligent acts or omission of others and assume full responsibility for my student’s participation in the Bonfire Night.

  3. My student and I acknowledge and understand that it is our responsibility to ensure that my student has safe transportation to and from Bonfire Night and further understand and acknowledge that the Protected Parties, as defined below, bear no such responsibility. My student and I knowingly and freely assume all risks, both known and unknown, relating to transportation from Bonfire Night, even those arising from the negligent act or omissions of others.

  4. My student and I, for ourselves, our heirs, executors, agents, successors and assigns agree to indemnify, defend and hold harmless the Hopkinton Public School District, Hopkinton High School, Town of Hopkinton and its officials, all Bonfire Night event volunteers and Hopkinton Parent Teacher Organization, including their employees, officers, members, volunteers, representatives, agents, heirs, executors, successors, insurers, and assigns (collectively, the “Protected Parties”), from and against any and all actions, claims, demands, complaints, suits, losses, damages, causes of action, responsibility, and liability for injuries, illnesses, losses, and/or damages, including but not limited to personal injury (death and/or disability), bodily injury, exposure to communicable diseases, viruses, bacteria or illnesses, property damage, and/or costs of any kind or amount whatsoever, which is caused by or results from, directly or indirectly, my student’s participation in this Bonfire Night, whether caused in whole or in part by the negligence of any or all of the Protected Parties, or otherwise to the fullest extent permitted by law.

  5. My student and I, for ourselves, our heirs, executors, agents, successors and assigns, hereby release, waive, discharge and covenant not to sue the Protected Parties, for any and all actions, claims, demands, complaints, suits, losses, damages, causes of action, responsibility, and liability for injuries, illnesses, losses, and/or damages, including but not limited to personal injury (death and/or disability), bodily injury, exposure to communicable diseases, viruses, bacteria or illnesses, property damage, and/or costs of any kind or amount whatsoever, which is caused by or results from, directly or indirectly, my student’s participation in this Bonfire Night, whether caused in whole or in part by the negligence of any or all the Protected Parties, or otherwise to the fullest extent permitted by law.

  6. I have read and understand this Acknowledgement & Assumption of Risk, Release and Waiver of Liability, and Indemnity Agreement and it is my express intent that this Agreement shall bind the members of my family and spouse, my heirs, executors, agents, successors and assigns, and shall be deemed as a release, waiver, discharge and covenant not to sue, and agreement to indemnify, defend and hold harmless the above named Protected Parties. I also understand that my student and I have given up substantial rights by signing it and sign it freely and voluntarily without any inducement.

CONSENT TO TREAT AND MEDICAL INFORMATION RELEASE

 

Permission is hereby granted by the undersigned to the Protected Parties to proceed with any needed medical treatment or emergency medical care, dental care and/or hospitalization (“medical care”) in the best interest of my student in the event he/she becomes ill and/or injured while participating in Bonfire Night . I further expressly authorize the Protected Parties to obtain and release medical information in the course of medical treatment and for the purpose of processing insurance claims. I understand that I am responsible for all expenses incurred for medical care provided to my student while at, or resulting from attendance at, the Bonfire Night.

PHOTO RELEASE


I give my permission to the Friends of the Class of 2027 Parent Volunteer Committee to use any and all photos taken of my student while at Bonfire Night for advertising (e.g., print ad, social media post), sponsorship acknowledgement, display at other Class of 2026 events and website content.

 


Parent/Guardian - Signatory must be 18 years of age or older.



PARENT AGREEMENT

 

I hereby represent and certify that I:

  • Am the custodial parent/legal guardian of the student identified herein;
  • Have legal authority to execute the Agreements in this registration form on my own behalf, on behalf of my student, and on behalf of my family;
  • Provided complete and accurate information throughout this registration form;
  • Have read, understand, and have explained to my student the contents of this registration form and the agreements contained herein, and agree on behalf of myself and my student to be bound by the terms of such registration form and the Agreements herein; and
  • Accept any potential disciplinary action that may be taken if my student violates any of the Rules & Expectations, as set forth above, and will be available to pick up my student if my student is asked to leave Bonfire Night for any disciplinary infraction or health concern.

I hereby give my student permission to attend and participate in any activities at Bonfire Night.

STUDENT AGREEMENT

 

I, the student, hereby represent and certify that I:

  • Have read, discussed with my parent/guardian, understand and agree to be bound by the terms of this registration form and agreements herein; and
  • Understand there are consequences for violating any of the Rules & Expectations above and agree to abide by any disciplinary actions imposed for such a violation.
